Goro Saizen (played by Toshio Karasawa) and Shuji Saito (played by Yosuke Eguchi) are doctors who interned at the same time, but they ended up on completely different paths. Saizen becomes the de facto top professor in the surgical department due to his exceptional personal skills. Saizen's aggressiveness makes the retiring head of the first surgical department, Professor Toh Masakazu (played by Koji Ishizaka), feel threatened, prompting him to consider others for the position. Consequently, the ambitious Saizen seeks assistance from his father-in-law—director of the Saizen Obstetrics and Gynecology Clinic, Yoi Saizen (played by Toshiyuki Nishida), and the head of the first internal medicine department, Kitsu (played by Yasuaki Ibu). Through bribery and forging alliances, he faces numerous challenges and ultimately secures the position of head of the surgical department. Meanwhile, his contemporary, Shuji Saito, an assistant in the first internal medicine department, is a practical and dedicated researcher. Their divergent choices lead to vastly different fates. Positioned at the pinnacle of his dreams, Saizen finds himself exposed to a bitter cold, resulting in tragedy. This series is adapted from the renowned novel by Toyoko Yamazaki, regarded as a "national novel of Japan." It has been remade five times and adapted for the screen multiple times.
