The story takes place in 1963 during the Cold War, where a mute woman named Elisa (played by Sally Hawkins) works as a janitor in a government laboratory. A serious illness took away Elisa's voice when she was young, and she has lived a solitary life in silence ever since, with only an elderly painter (played by Richard Jenkins) and colleague Zelda (played by Octavia Spencer) as her friends. One day, a high-security alarm is triggered in the laboratory, and a mysterious tank filled with water is brought in. Elisa is shocked to discover that a strange half-human, half-fish creature is kept inside. Led by Richard (played by Michael Shannon), the team of scientists aims to extract a substance from this creature that can be used to create biological weapons, but to Elisa, it is just another solitary being like herself. As time goes by, Elisa and the creature develop feelings for each other.
