John Q. Archibald (played by Denzel Washington) is a factory worker struggling to make ends meet. As the couple's relationship deteriorates, their marriage is on the brink of collapse. To make matters worse, their 9-year-old son Michael faces an unfortunate crisis. One day, Michael suddenly collapses at the baseball field. When John Q takes his son to the hospital, he learns that Michael has a severe congenital heart condition and needs a heart transplant to survive, a procedure far beyond the scope of his health insurance. After several failed attempts to communicate with the hospital, the administration decides to remove them from the transplant waiting list. This action infuriates John Q, who stormed into the emergency room with a gun, taking innocent medical staff and patients hostage, demanding that the hospital perform the surgery for his son immediately. This moral and legal battle of humanity is far more complex than it seems...
