Sherlock Holmes (Basil Rathbone) and Watson (Nigel Bruce) travel to Quebec, Canada, invited to attend a secret meeting of the Canadian royal elite. However, they quickly find themselves embroiled in a series of horrifying murder cases. Local residents tell tales of a mysterious creature with red lips lurking in the marshes, but Holmes is skeptical, believing that the real culprit may be disguising themselves as the monster to obstruct the investigation and confuse the truth. Holmes understands that the truth may be hidden among the villagers; he speculates that the murderer could very well be disguised as an ordinary person. To uncover the mystery, he embarks on a bold scheme, placing himself as the next target of the killer. This move is both a dangerous game and a peak challenge to his intellect and deductive abilities. Amidst the legends of the red-lipped creature and the actual cases, Holmes struggles to dissect the truth, utilizing his unique insights and reasoning skills to clear the fog.
