After a busy day, young nurse Karen (played by Ilona Elkin) in the psychiatric hospital ward takes the last subway train home. However, the subway suddenly stops in the middle of the tunnel, and a bloody massacre unfolds in the carriage. A group of fanatical cultists launches a brutal slaughter of the passengers under the guise of saving souls. Karen and other survivors manage to escape the carriage, but the bloodthirsty attackers relentlessly pursue them.
