Tangshan (played by Donnie Yen) returns to China after studying medicine abroad and opens a Western medicine clinic with his assistant Liu Guobang in a slum area of Zhabei, Shanghai. The local Ax Gang bully, Yu Laoqi (played by Yu Rongguang), frequently brings people to beat up villagers in order to acquire a piece of land. Tangshan and Liu Guobang step in to stop the violence and provide free medical treatment to the injured villagers. To put an end to Yu Laoqi's actions, Tangshan visits the land's owner—his former teacher Simon, whom he studied under in England. Tangshan secures the villagers' land by making a deal with Simon. Yu Laoqi's sister, Yu Xiaoqian (played by Zhu Yin), has suffered from a throat ailment for many years and cannot speak. Believing in fox spirits, Yu Laoqi rejects Western medical treatment, hoping that the spirits can cure his sister. By chance, Xiaoqian meets Tangshan, who, through his sincerity, convinces her to agree to surgery. Meanwhile, children in the slums begin to go missing, and Tangshan discovers that the masterminds behind this are the Ax Gang and Simon, leading to an inevitable fierce battle.
