Tom (Nick Nolte) is a typical Southern young man in America, whose troubled childhood experiences have turned him into a silent and cynical man, completely at odds with the insincerity of the world around him. The news of his brother's death plunges Tom's already chaotic life into deeper despair, putting significant strain on his relationship with his wife, Sally (Blythe Danner). Tom has a sister named Savannah (Melinda Dillon) who is struggling to make it in New York, but instead of success, the news that reaches them is that Savannah has attempted suicide and is now in a mental hospital. Susan (Barbra Streisand) is Savannah's psychiatrist, and she discovers that hidden in Savannah's silence is the key to her healing.
